- Western/Rodeo Camp Registration Form
- Camper Information / Release of Liability Form
2. Fill out forms completely and sign, include your payment by check, Visa or Mastercard. If you pay by check mail to the address below or drop of at the Morning Star Farm office. If you pay by credit card you can fax the forms to 897-8306 or email to contactmsf@aol.com

You will get a confirmation call when we have received your forms and payment. No written confirmation will be sent.
Cancellations & Refunds- Refunds - less a $50 booking fee - for cancellations up to May 31st, thereafter, No Refunds unless space can be filled from our waiting list, then above rule applies.
- Substitutions - After you receive a confirmation call, you have a 48 hour grace period to make any changes. After this time a $40 fee is required for any substitutions of camper or camp date. (Subject to availability)
